If you are using Internet Explorer 8 and do not see the "Accept" button at the end of the license agreement, go to the browser’s "Tools," select "Advanced" and then check the box at the bottom next to “Automatically recover from page layout errors with Compatibility View. Alternatively, you may want to try using a different browser.

If you are using Safari and see a “Group/Practice is required” error on the User Information page, please use Chrome or Firefox instead.

If you see an error stating “Unable to find an account connected with your IP address…” please follow the instructions to Clear your browser’s cache and cookies.
